Insurance
If you have a PPO plan with out of network coverage, services may be covered in full or in part by your health insurance or employee benefit plan. HMO insurance will not provide coverage for services provided at this facility. Please check your coverage carefully by asking the following questions:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is it and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- What is the coverage amount per therapy session?
- Do I need written approval from my primary care physician in order for services to be covered?
Payment
I accept cash, check or we work out a payment plan that works for you. After each session, you will receive a super bill you can submit to your insurance. Insurance will be reimbursed directly to you from your carrier.
Cancellation Policy
If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
